About Johns Hopkins Home Care Group

More than a century ago, Baltimore merchant Johns Hopkins did more than create a university, a hospital and a medical school. He provided a vision of a unique, academic-based health system with a focus on education and compassion - a place where the quest for new knowledge would result in more effective and caring treatment for all.

As a component of Johns Hopkins Medicine, Johns Hopkins Home Care Group continues that vision by providing comprehensive home health services, home infusion, respiratory and medical equipment, supplies and outpatient pharmacy medications to patients in Maryland, Washington, D.C. and Northern Virginia.

What We Do

Johns Hopkins Home Care Group (JHHCG) is a diverse and inclusive, fully integrated member of Johns Hopkins Medicine (JHM) serving as an integral component of the healthcare continuum by supporting the JHM mission of safe, quality patient care, education, and research. JHHCG strives to be the preeminent home care provider to the Johns Hopkins community by integrating comprehensive home care service and products through a single point of access, utilizing clinical expertise and exhibiting outstanding service excellence. Johns Hopkins Outpatient Pharmacy, which is part of JHHCG, provides community and specialty pharmacy services for the Johns Hopkins Community.

 The community pharmacy residency is committed to providing residents with the skills and knowledge necessary to become exemplary community pharmacists providing care for a diverse group of patients.  Upon successful completion of the residency, the pharmacist will have developed the clinical, analytical, and leadership skills necessary to function as a clinical practitioner in an integrated health-system, a community pharmacy setting or pursue a PGY2 residency.

This program emphasizes the development of advanced practical skills needed by community pharmacy practitioners concentrating on enhancing existing clinical services and implementing pharmacy practice programs. The experience is individualized to each resident to develop clinical, analytical, teaching, and leadership skills. Residents will have opportunities to provide patient care services including medication therapy management, immunizations, and compounding. The residents will work in a collaborative environment with exposure to diverse clinical specialty areas, including solid organ transplant, infectious disease, geriatrics, oncology, and transitions of care. Opportunities to precept students and provide Accreditation Council for Pharmacy Education (ACPE) programs are available. Collaboration with providers in an integrated health system will be emphasized through trans-disciplinary care to improve patient outcomes.
