Evolent Health is engaged in healthcare delivery and payment. We enable value based healthcare: higher quality, lower cost healthcare. Evolent supports health systems and physician networks in their transition to value based healthcare and population health management through People, Processes and Technologies.
Evolent provides an end to end technology enabled services platform for providers. The platform is powered by its technology processes and integrated services, which enables providers to migrate economic orientation from fee-for service reimbursement to payment models that reward value based payment models. Each hospital system is a client of ours – and we consult and advise them in their transition. We entrench ourselves into the transition – we bring in implementation teams and transformation teams that create care management programs and integrate our EHR system with the client’s system. We also take on some of the same risks as our clients throughout the transition – which differentiates our business offering.
